In Photographs, the Head Is Always Tilted to One Side
The short answer
When the pelvis or the shoulders are twisted, the body makes its final adjustment at the head so that the eyes stay level. But the neck is unusually densely packed with sensors that measure position, so a head held tilted can send information that disagrees with what comes from the inner ear and the eyes. The place that holds posture and the place that reads how the body feels lie closer together than one might expect.

Many people notice it in an ID photograph or a group picture. They thought they were standing straight, and yet the head is always tilted to one side.
They come in thinking the neck must be stiff. I do not look only at the neck. The head is usually the last thing to tilt.
When the lower body twists, the head makes the final correction
If the pelvis sits low on one side, or one shoulder rides higher than the other, everything stacked above it goes slightly out of line in turn. Left alone, that would leave the field of vision tilted.
But we need to see the world level. So the body makes a last correction at the neck, tilting the head slightly the other way to bring the eyes back to horizontal. The tilt that shows up in a photograph is often not where the problem began but where something that began lower down was finished off.
The neck measures unusually densely
Inside a muscle are sensors that register how much its length has changed. They are called muscle spindles. They are not spread evenly through the body.
Just below the back of the skull, small muscles bridge the head and the first vertebra of the neck. These are the suboccipital muscles. Measured for spindle density, they run from about 100 to 240 per unit of muscle weight. The muscle that brings the thumb across the palm has about 17. That is 6 to 14 times as many.
It makes sense. These muscles are not there to produce much force but to report which way the head is currently facing. The head carries the eyes and the balance organs of the inner ear, so how it sits relative to the trunk has to be known very precisely.
So the information can disagree
At least three streams feed the body's sense of its own posture: the balance organs of the inner ear, the eyes, and position information coming up from the neck. When all three give the same answer, there is no trouble.
When the head is held tilted for a long time, the answer from the neck can drift slightly out of step with the others. Among those who tell me they feel dizzy or oddly untethered, some have a neck and shoulders that are always tight.
Here, though, I have to speak carefully. Dizziness attributed to the neck remains a debated idea, and there is no test that confirms it. It is something to consider only after other causes have been ruled out. So I hold it as a possibility rather than a conclusion.
I also look at where that information arrives
The side that receives the incoming information and holds posture has a name too.
At the middle of the cerebellum is a part named for its resemblance to a worm, the vermis. When its function is briefly suppressed in people and measured, the sway of quiet standing increases straight away, with the eyes open or closed. Which means it had been holding the body all along.
There is one more place worth noting. One of the brain regions that receives balance information sits close to an area called the insula. The insula is also known as a place that reads what is happening inside the body — the beating of the heart, for instance. There are reports that how finely someone reads their inner state moves together with feelings of unease.
Handling posture and handling how the body feels take place closer together than one might expect. When someone speaks of dizziness and of feeling unwell in the same breath, I keep this in view.
When the trunk itself is twisted
Where the spine curves sideways, the story becomes clearer. In a study of adolescent scoliosis, those affected were less accurate at judging what angle their own lower back was at when bending and straightening, and they swayed more in standing. Much of that sway was accounted for by the errors in sensing.
It is not only a matter of the angle of the curve. The measuring itself is unsteady.
So this is what I look at
Even when someone comes in for neck pain, I first look at which way the head is tilted and by how much. Then I work downward to see where that tilt came from.
It is the same story as looking at the foot for back pain, told from the other direction. That article travelled from the foot upward; this one travels from the head down.
When to be seen first
Dizziness includes things that need urgent attention. If it comes on suddenly and severely, or with slurred speech, weakness on one side, double vision, or difficulty walking — emergency care comes before any of this. And if a spinal curve in adolescence is visibly increasing, that too should be seen first.
Korean medicine always asked whether the two sides matched. Which side you lie on, which shoulder rides higher, which side stiffens first. It never looked at the painful place alone; it looked at how the whole body was standing.
Today that question can be given names. The neck is densely set with sensors that report position, and the body is at ease only when what they report agrees with what comes from the eyes and the inner ear.
That the place which hurts and the place to work on may differ — this is a view the medicine has held for a long time, and it is where I practise.
